- Post Physical: Athletes must upload their front page (Pre-Participation Physical) and the doctors page (Physical Examination) into their current Dragonfly account (Returning athletes should not make a new account). We will request that each athlete take a picture of their form with their cell phone. We recommend that parents also take a picture of this form for their own digital records in case the paper is lost or damaged.
